A member who, pursuant to the second paragraph of section 60.5 of the Professional Code (chapter C-26), denies a client access to the information contained in a record made in his regard shall inform the client in writing that the disclosure would be likely to cause serious harm to the client or to a third party.
He must also, in the same document,(1) identify the serious harm to the client or to the third party concerned;
(2) identify the third party concerned.
